What the Research Says
Fish have nociceptors — sensory receptors that detect potentially damaging stimuli. The scientific debate isn't really about whether fish have these receptors (they do) but about whether fish experience pain in a conscious, subjective way the way mammals do. The fish brain lacks the neocortex that processes pain experience in humans and other mammals, which is why the question is genuinely complicated.
What's less debated: fish respond to tissue damage with behavioral changes. A fish that's been hooked will often behave differently — more cautiously, less willing to strike — for a period after the event. Whether that's "feeling pain" in a meaningful sense or a simpler stress response is where the science gets murky.
For practical fishing purposes, the more useful question is: does a hook that breaks off cause lasting damage?
What Happens to a Hook Left in a Fish
This depends heavily on the hook material and where it's lodged. Modern fishing hooks are made from high-carbon steel or stainless steel. High-carbon steel hooks rust and dissolve relatively quickly in freshwater — typically within a few weeks to a few months depending on water chemistry. A high-carbon hook lodged in a fish's lip or jaw will usually rust away before causing significant long-term damage.
Stainless steel hooks are a different story. They don't rust, which means they can remain in a fish indefinitely. This is one reason many conservation-minded anglers avoid stainless hooks for catch-and-release fishing, particularly for deeply hooked fish.
Location matters as much as material. A hook in the lip or jaw — the most common hookup location — is unlikely to cause serious long-term harm even if it stays in. A hook in the gills, throat, or stomach is more serious because it can interfere with feeding and breathing.
What I've Observed on the Water
I've caught fish with old hooks already in them. Not rarely — fairly regularly over years of fishing. Most of these fish appeared healthy and were feeding normally. The hooks were usually rusted to the point of being barely recognizable, lodged in the lip or jaw, and clearly hadn't prevented the fish from living a normal life.
I've also caught fish that were clearly hook-shy — fish that would follow a lure repeatedly without committing, or that would strike and immediately spit the lure before the hook could engage. Whether this behavior is a learned response to previous hooking or something else is hard to know, but it's consistent with the idea that fish retain some memory of negative experiences.
The Practical Takeaway
If you break off a fish with a hook in it, the outcome depends on where the hook is and what it's made of. A high-carbon hook in the lip will almost certainly rust away without causing lasting harm. A stainless hook in the throat is more concerning.
The best practice for catch-and-release fishing is to use high-carbon hooks, barbless or with crimped barbs when possible, and to cut the line as close to the hook as possible if you can't remove it safely. Trying to dig out a deeply embedded hook often causes more damage than leaving it.
